Original abstract

The paper provides the discussion of possible influence of tympanic membrane graft material on sound transfer function. The eardrum perforation and the result of myringoplasty was implemented in our comprehensive three-dimensional computational model of the human ear. The large scale of computations is presently the biggest obstacle to progress in our ear mechanics modelling. The results demonstrate the promise of large scale computational modelling approach to the study of ear mechanics and to the improvement of otosurgery effectiveness. From the point of view of TM graft material is recommended silk because for it (in comparison with the normal ear) there was observed the increase of sound transfer function in all audio frequency range, in the region of 1-3 kHz this enhancement achieved 3 dB. Presented results should contribute to the future design of controlled clinical experimental studies in otology especially in the procedures of myringoplastics and tympanoplastics.

Czech abstract

Je diskutován vliv materiálových vlastností záplat používaných v ušní chirurgii při myringoplastikách na sluch. Matematické modelování provedené na kompletním modelu ucha ukázalo na výrazné změny v modálních frekvencí a ve tvarech kmitů bubínku při změně tuhosti materiálu záplat. Z hlediska přenosu akustických signálů do vnitřního ucha se přitom jeví výhodné používání záplat z přírodního hedvábí, které jednak vyhovuje po stránce hojení a jednak vykazuje proti poměrně měkkým materiálům (kůže, syntetický kaučuk) teoretické zvýšení hodnot přenosové funkce o 3 dB v pásmu 1-3 kHz.
